Teleform

Teleform is a computer-based system available through the PCRI central office that provides the means to produce, analyze, and report evaluative results via a scanning technique.  Advantages of this software are many:

  • Forms Design: Each form can be designed for data collection according to unique and individualized specifications; please see the attached example.
  • Forms Distribution: Forms can be transmitted by Web-based means, faxed, or printed regarding use to collect data.
  • Forms Interpretation: Completed forms can be scanned, faxed, or submitted electronically.  Teleform software provides highly accurate recognition capabilities to identify and interpret incoming pages.
  • Data Verification: Unrecognized or invalid entries can be identified and displayed for high-speed correction processing.
  • Data Storage: Data will automatically be stored in the appropriate data format with little or no human intervention.
  • Data Analysis: Data can be analyzed via easy transport into common statistical packages.

These capabilities greatly enhance our research capabilities.  Such use facilitates population-based data collection and analysis.  To secure Teleform services, please contact the PCRI central office (614) 293-8007.
